St. Mary's emergency hall blazed with fluorescent light. Disinfectant stung my nose.

I handed over the check at the payment window, signed my name with shaking hands.

Deposit: $85,000. ICU account reserve: $100,000.

The screen flashed green: Payment Successful.

The red "Surgery in Progress" light above the closed doors lit up.

All my strength drained away.

Four and a half hours.

I sat on the cold metal bench outside the OR like a corpse, staring at the harsh white ceiling lights.

At 4 a.m., the automatic doors slid open. The head surgeon pulled off his mask, walked out, and patted my shoulder tiredly. "Surgery went perfectly. All four bypass grafts successful. Patient transferred to cardiac ICU. She's out of immediate danger."

The moment I heard those words, the wall I'd held up for twenty-four hours collapsed.

Before I could thank him, everything went black. I crumpled toward the cold floor.

Nurses shouted. Then all the lights went out.

I woke to the smell of glucose and saline.

I was in an ER observation bed. An IV dripped into my left hand.

A faint grayish light seeped through the window. The rain had stopped.

The curtain pulled back. The on-call doctor held a stack of freshly printed lab reports. She sat down beside me, expression complicated. "Ms. Caldwell. You're awake."

My lips were cracked. "Doctor... how's my mother?"

"Your mother's vitals are stable. Cardiac enzymes dropping. Don't worry about her." She adjusted her glasses and handed me the report, voice low. "You need to worry about yourself."

I looked down.

At the bottom of the report, two lines of bold black text slammed into my vision.

[Human Chorionic Gonadotropin (HCG): 14,228 mIU/mL]

[Ultrasound: Intrauterine early pregnancy, single fetus viable, embryo size approximately 7 weeks gestational age]

"You're pregnant. Seven weeks."

The doctor looked at my ashen face, voice grave. "Severe malnutrition. Severe anemia. Plus today's trauma and emotional shock—you have threatened miscarriage with hematoma. If you want to keep this baby, you must rest completely starting now. If you don't want it, given your condition, immediate evacuation is your only option."

Seven weeks... six weeks ago...

That wild Paris night. The kiss on the street. Every time he filled me completely, his hot breath, the Italian words whispered in my ear...

This was Marcelo's child.

The child of the mafia boss who'd just thrown a check in my face and told me to disappear forever.

"I... I need to think."
